DigiCare4You’s Work Package 2 is responsible for the stakeholder engagement, situation analysis & implementation strategies of the project. As part of this, partners from Sant ´Anna School of Advanced Studies developed a situation analysis to record and understand the contextual factors related to primary and community healthcare services in each of the four implementation countries of the DigiCare4You study (Albania, Bulgaria, Greece, and Spain).

Investigators from La Trobe University, Baker Heart and Diabetes Institute, Ghent University, NYU Grossman School of Medicine and Meteda conducted a literature review on the evidence of both clinical- and cost-effectiveness of mHealth and community-based interventions for the prevention and management of Type 2 Diabetes mellitus (T2D) and Hypertension (HTN). 

An active lifestyle with regular low, moderate and high intensity exercise is important for everyone and can help prevent and optimally manage chronic conditions such as diabetes and hypertension.

In April 2021, the Digicare4You project held its kick-off meeting, officially launching a five-year multistakeholder Horizon2020-funded project. The 16 consortium members from Europe, the US and Australia gathered online for two days to mark the start of the project and to delimitate the scope of each member´s work amongst the nine different work packages of the project.
